Yes. We’ve included the link to the New Client Intake Form below. Simply click on the link, print the PDF file and you’ll be able to fill it out in advance and bring it with you to your first appointment. If you prefer to fill the form out in our office, please arrive early enough to give yourself time to complete the form before your scheduled appointment.
